FIU Forms (forms.fiu.edu) Deactivation Notice

New form creation will be disabled by the end of 2023. The FIU Forms application will be retired on May 10th of 2024. Prior to the deactivation of FIU Forms, all users should backup their form data and find alternative options for their forms.

Listed below are several options for building web forms that are available to the FIU community.

If you would prefer to continue using the Current Forms application, please use the form below to make a request for a stand-alone, custom installation for your department or group. We will begin implementing these installations in Spring 2024. There will be an annual hosting and maintenance fee of $750 for these installations. Users will also be required to attend training on how to properly secure their forms and form data.
